New patients

Your first visit,
without the guesswork.

Everything you need before you walk in - what to bring, what we will ask, and the packet you can fill out at home.

How it works

Three steps to your first appointment.

1

Book or call

Pick a time yourself with our online booking, or reach the office at 772‑210‑2037, Monday through Friday, 9 AM to 5 PM. Let us know if you were referred by another provider.

2

Fill out the packet

Download and complete the new patient packet at home. Filling it out in advance gives us more of your visit for actual conversation.

3

Arrive a few minutes early

Bring your completed packet, photo ID, insurance cards, and your current medication list. We will take it from there.

Before you come in

The New Patient Packet

Print it, fill it in, and bring it with you. It covers your demographics, insurance, consents, medical and pulmonary history, medications, and allergies - the same information we would otherwise collect in the waiting room.

What to bring

  • Photo ID and insurance card or cards
  • A list of current medications and dosages
  • Referral or recent records from other providers, if applicable
  • Your completed packet
Inside the packet

What we will ask, and why.

Name, date of birth, address, phone, email, employer, preferred pharmacy, and an emergency contact. Your preferred pharmacy lets us send prescriptions the same day.
Primary and secondary insurance, policy and group numbers, and an assignment of benefits authorizing payment directly to the practice.
  • Consent to treat - permission to evaluate and treat you.
  • HIPAA acknowledgment - confirming you received our Notice of Privacy Practices.
  • Medical records release - so we can request and share records for your treatment.
  • AI scribe consent - entirely optional. You may consent to AI-assisted visit documentation, or decline. Declining does not affect your care.
Past conditions, surgeries, family history, and social history. The pulmonary section asks about smoking and vaping history, sleep apnea, CPAP or BiPAP use, workplace and environmental exposures, and pets - all of which change how we read your symptoms.
Every medication with its dose and frequency, plus any allergen and the reaction it causes. Bringing the actual bottles is welcome if that is easier than writing them out.
A short symptom checklist - cough, shortness of breath, wheezing, chest pain, palpitations, fever, fatigue, weight loss, and swelling - with room to add anything else you have noticed.
Billing

Our financial policy, plainly stated.

We would rather you know the terms up front than discover them at checkout. The full policy is in the packet and requires your signature at your first visit.

  • Payment is due at the time of service
  • Copays, deductibles, and coinsurance are the patient’s responsibility
  • Insurance billing is not a guarantee of payment
  • Self-pay balances are due at the visit
  • A credit card on file is required
  • Missed appointments carry a $50 no-show fee
  • Unpaid balances may be sent to collections
